1. Process and required documents for reissue/fresh passport:

1. 

Fill online application form please click this link "https://embassy.passportindia.gov.in/". Please choose online registration option and select Mission: Indonesia – Medan.

2. 

After submitting the online form, please take print out of application, sign in the box, and paste 2 inch x 2 inch, light background with 80% face area and showing shoulders. Guidelines for photograph

3. 

Attach copy of passport (first page & last page), copy of KITAS/residence permit.

4. 

Please fill the details as per passport and attach proof, if details in any column are different from current passport or any change is required.

5. 

Attach copies of marriage certificate, Joint colored photograph and an Affidavit in case spouse name to be added

6. 

Attach copy of rent agreement, Aadhaar card, bank statement, election card, etc. (for changing address in India)

7. 

In case of loss passport, FIR is required. For non-resident of Indonesia, please refer to Misc. Consular Service (Sr. No. 16)  for issuance of Emergency Certificate

8. 

Copy of Aadhaar card, Pan card, if any

9. 

For re-issue of Minor’s passport attach copies applicant’s passport, parent’s (mother and Father) passports copies (front and back pages), KITAS copies along with originals and Declaration form from parents (consent for passport renewal) & Declaration form (if one of the parent staying abroad)

10. 

For new Indian Passport /Birth Registration for Newly Born Baby or if one of the spouse is other nationality please refer to Misc. Consular Service (Sr. No. 6)

12. 

Please bring passport and other related documents in original with one set of copies along with the original proof of payment at the time of submission at Consulate:19A, JL. Uskup Agung, Sugiopranoto, Medan – 20152 (Indonesia). Originals will be returned immediately after the verification except applicant’s passport.

Note: Passports are reissued on expiry of final validity (can be done one year before expiry), on exhaustion of visa pages, damage or loss, appearance/particulars change, etc.

4. PUBLIC ADVISORY: Indian citizens living in India and abroad are advised to apply for urgent re-issuance of their passports in the following cases:-

(i). HANDWRITTEN PASSPORTS

The International Civil Aviation Organization (ICAO) has set a deadline of the 24th November, 2015 for globally phasing out of all non-Machine Readable Passports (MRPs). From the 25th November, 2015 onwards, foreign Governments may deny Visa or entry to any person travelling on a non-Machine Readable Passport.

The Government of India has been issuing Machine Readable Passports since 2001. The Passports, however, issued before 2001 and particularly those issued during mid 1990s with a validity of 20 years will fall in the category of non-MRPs. All handwritten passports with pasted photographs are also considered non-MRPs. By the end of November 2014, it is estimated that there are approximately 2.86 lakh handwritten passports in circulation. Approximately 6 crore Indian hold valid passports. Indian citizens residing in India and abroad and holding such passports with validity beyond the 24th November, 2015 are advised to apply for re-issue of their passports well before the deadline in order to avoid any inconvenience in obtaining valid visa or international travel.

(ii) PASSPORTS VALID FOR LESS THAN SIX MONTHS

Many international travellers may not realise that having an unexpired passport is sometimes not enough to obtain Visas or to enter certain foreign countries. Indian citizens travelling on Passports which may expire in less than six months should renew their passports before any upcoming international travel. It is especially important to check the passports needs of any minor who may be accompanying their parents as passports for minors have a shorter validity period (5 years) than passports for adults (10 years). The universal practice in vogue now is; “Once your passport crosses the nine-year mark, it is time to get new passport”.

(iii) EXHAUSTION OF VISA PAGES
